FAQ

  + Medical Insurance?

    We participate with many insurance companies and managed care plans. Please
    check with your insurance company about your coverage. To process your
    billing, provide us with your current primary and / or secondary insurance
    information. It is your responsibility to provide the office with the most
    current insurance and demographic information and to have an understanding of
    your insurance benefits and coverage.

  + Vaccines for Children Program

    This program is paid for by the state if you do not have commercial insurance
    coverage or you are a Medicaid recipient you are eligible for the VFC
    program. Please inform the nurse of your insurance status before the vaccines
    are given. If your insurance does not cover vaccines you can contact The
    Cuyahoga County Board of Health at 216-201-0241 or www.ccbh.net to find the
    nearest vaccine clinic.

  + Payment?

    Co-pays must be paid at the time of your visit per your contractual
    arrangement with your insurance company. Depending upon special
    circumstances, other financial arrangements may be made prior to your
    appointment through our billing department at 440-356-3108. In addition a
    $2.00 fee will be added when a known co-payment is not paid at the time of
    service. We accept Visa, Master Card, Discover, Debit Cards and personal checks

  + Appointments / Hours ?

    Patients are seen by appointment only, including sick children. Our schedule
    offers morning, afternoon and evening appointments Call as soon as possible to
    arrange an appointment. If you need to add a sibling to an already scheduled
    appointment please call to allow additional time. We do leave time open each
    day for emergencies and acutely ill patients. Please call 24 hours in advance if
    you must cancel an appointment. We understand that sometimes delays happen
    therefore we respectfully ask that you call the office to notify us of your
    delay. We respect your valuable time and do our best to avoid delays.

  + Meet and Greets?

    All of our doctors are available for meet and greet appointments. Feel free to
    call us to schedule your complimentary appointment to meet and interview any
    or all of our doctors and staff at in the Fairview Park office at 440-356-2272.
    To set up meet and greet with Dr Mary Murphy in our Strongsville
    office you may call 440-846-7481.

  + Policy for Financial Responsibility?

    The person who brings the child in and signs the Patient Consent Form
    will be considered the responsible party. Billing statements will be
    sent to this person.
    If there are any stipulations you may contact our patient accounts
    department to discuss any legal changes.

  + Collections?

    If you have been notified your child / children’s accounts have been put into
    the collection status they will not be allowed to be seen in our office until the
    balance has been paid or a satisfactory payment arrangement has been made
    and honored on a monthly basis.

  + Prescriptions / Refills?

    If your child is on long term medications, refills will be handled at the time
    of your visit.
    On some occasions it may be necessary for you to phone the office for refills
    this should be done only during office hours. Please have the name and number
    of your pharmacy ready when you call. Refills of controlled medications need to
    be picked up at the office and will be ready in 2-3 days.

  + Referrals?

    Please check with your insurance company to see if you require a referral. If a
    referral is needed it is your responsibility to verify if the physician or the
    facility is in your insurance network. After you have made your appointment
    with the specialist or facility you need to contact our office with your
    appointment date and who or where the appointment is. Referrals are done
    within 7-10 business days.

  + School and Sports Physical Forms?

    School and sports physical forms can usually be completed if your child has had
    a physical from one of our physicians within one year. You can call to confirm
    this information. Please complete your portion of the form and deliver to the
    office. Your form should be completed and ready for you to pick up in two to
    three days. We can return via mail if you have included a self addressed
    stamped envelope. If you are bringing the form in at the time of your child’s
    physical exam, complete your portion ahead of time. The physician portion will
    be completed at the time of service.

  + Medical Records?

    Transfer of medical records must be requested in writing please include the
    name and address where the records are to be sent. We do not fax
    records. There is a $15.00 processing fee per chart. We will send medical
    records 2 to 3 weeks after the processing fee has been received.
    Copies of immunization records are available at your request. Forms are
    available in our office to request medical records from previous physicians.

    What are my patient responsibilities?

    •   Providing complete and accurate information about past
        and present medical conditions, hospitalizations and medications.
    •   Reporting changes in conditions and treatments
    •   Inquiring about diagnosis and treatment if clarification is needed
    •   Following advice and treatment plan
    •   Being on time for appointments and or canceling in a timely manner
